Services rendered to patients in California are rendered by Chuck R. Colas, D.O., P.C. Services rendered to patients in NJ, DE and AK are rendered by My Catholic Doctor Medical Group, PC. Services to patients in all other state are rendered by My Catholic Doctor, P.L.L.C. All rendered services are intended to be limited to services permitted under Roman Catholic doctrine. The MyCatholicDoctor Foundation is a 501(c)(3) nonprofit organization and all donations are tax-deductible. Subject to budgetary constraints, the Foundation provides funding for care for indigent individuals wishing to obtain faithful Catholic services from telehealth entities which are part of a national Catholic telehealth network which meets the Foundation’s criteria for what constitutes faithful Catholic healthcare. Any such entity may apply for inclusion in the Foundation’s funding.
